e-health, self management support, e-health applications, behavior change
As an effect of increased living standards and improved health care, life expectancy has risen in many parts of the world. At the same time, welfare diseases such as diabetes and obesity increases. As the number of elderly and long-term sick rises, so will the medical care needs. E-health applications are often presented as a way to meet future medical and doing so within the scope of existing resources. By facilitating health communication and widen access to health services by offering them online, e-health applications encourages people to become more engaged in their own health, thus working proactively towards a healthier population. However, e-health applications often fail to reach their full potential. The purpose of this study is to explore how health care providers and health care recipients perceives e-health applications and their usefulness and thus being able to identify factors significant for successful introduction and use of e-health services. Two e-health applications have been analysed and six important aspects have been identified and discussed. In order to be successful, an e-health application must facilitate behaviour change and be integrated in the everyday life of the user. Substantial and adequate evaluation is essential to make sure that the application meet the requirements from both health care providers and health care recipients. An understanding of the possibilities technology has to offer is needed in order to fully exploit the potential of e-health applications in health care. Instructions for health care providers on how to use the applications are essential not only to ensure their proper usage but also to make sure that applications are being presented to recipients in a satisfactory way and that care providers can offer the support and help recipients may need. Finally, when discussing e-health applications it is important to remember that there are people who does not want to get involved and that there is still a demand for face-to-face interaction in health care. Therefore, e-health services must be complemented with alternatives providing different types of interaction opportunities.

Co-creation, co-design, healthcare, innovation, drama-driven workshop
Healthcare in Sweden are in need of a drastic change to meet the requirements of highly developed societies, and information technology is seen as an essential part to get there. However, the Swedish healthcare have to challenge their traditional innovation approaches as well. For example, it has become more common to engage patients in the innovation process through web-based platforms. We have been focusing on an approach called co-creation by studying a workshop, in which potential users and other stakeholders are cooperating creatively to come up with innovative solutions in order to improve healthcare practice. Our findings suggests that co-creation may increase the mutual understanding between different stakeholders, for example healthcare professionals and IT developers, enable horizontal learning through and outside the organization and also to raise concepts and actions that are responding to healthcare needs. However, we have found important challenges in bridging barriers that may occur when gathering people of different competencies, and involving proper participants that are representative of their groups.

The effectiveness of communication in the Swedish healthcare system context has long been the subject of debate. Issues stemming from usability, accessability and interpretation affect the lives of patients, and yet the vocal groups in the healthcare communication debate are patients and administrators, not healthcare professionals. With the advent of eHealth services and new demands being put forth on the ability of healthcare professionals’ to handle increasingly numerous and innovative systems, it is of more importance than ever to understand the healthcare professionals’ work context. This article is based on interviews with five healthcare professionals concerning their views on electronic health records (EHR), eHealth services, trends in healthcare services development in Sweden, as well as their views on the future of eHealth solutions and health information technology (HIT). Analysis of collected information shows an apprehension among healthcare professionals towards electronic communication, as well as a dissatisfaction with the usability of modern healthcare systems. Sentiments are generally favourable towards eHealth solutions however, and most negative criticism of modern systems appears to stem from a frustration with being given less time with patients, as well as fears of compromising patient integrity. Results point toward healthcare professionals’ being an untapped resource in the context of healthcare communication development, though major changes would have to occur in the distribution of demands on the healthcare professionals’ time, as well as a shift in the way we view healthcare communication, from more traditional means, to modern, interactive systems.

Today is what you could call a golden age for technological development in health care. Technology is given a bigger role in healthcare and serves not only as a support for health professionals in their daily work but also as a tool for the patient. With the help of eHealth services, patients are becoming more involved in their own care. This improves patient’s knowledge about their health, which in turn contributes to higher motivation to perform self-care. Since the autumn of 2012, health records online have been implemented in certain counties through an eHealth application named ”my online health record”. This application will soon be launched nationwide in Sweden. In this study we are examining how this technological progress can be understood from a number of actors’ perspectives, who have an interest in the situation. We have investigated this by performing a qualitative study in which we interviewed respondents from three different professions in order to find out their attitude towards the application. The result from the data collection shows a development towards a greater involvement of patients in their care process. Many doctors see this as a positive thing since it increases patient safety and improves the quality of the meeting between doctor and patient. There is a risk, however, of creating a digital gap between certain patients. A large group of patients have a high education, good skills within IT and therefore the ability to assimilate the information, however there’s a minority whom has no internet access or knowledge to assimilate the information in a good way. This creates a gap between these patient groups, where the first group will receive better care and the later gets left out of the system. In conclusion, the study has shown that this ongoing trend cannot be stopped, patients will demand access to more and more information in a more accessible way.

Today significant resources are provided to improve the digital work environment within the Swedish healthcare to create efficiency, good work environment and patient safety. Despite this, major flaws are shown by the lack of communication between the eHealthcare systems. This becomes a stress factor among healthcare professionals because of increased administration and decreased physical contact with patients. The purpose of this study is to create a understanding of the concept of interoperability at different levels in the eHealthcare systems. This has been conducted through a qualitative interview study at a healthcare center where the respondents have been describing their digital work environment and the way it works today. Our result indicates that there is a dissatisfaction among the health professionals about the way the eHealthcare system works in the current situation. This is mainly because of the low degree of interoperability in current eHealthcare systems, which indicates a lack of knowledge in the field of interoperability. From this we find it important to look at eHealthcare system as a part of an information ecology instead of seeing it as a separate IT artifact.
